Team On-Line USFF Registration

1. Click on the preceding link. It will take you to the Welcome to “U.S. Futsal Team Registration” screen.
• Make sure that you have the id for the coach before you proceed any further. If you are registering an assistant coach then make sure you have their id as well.
2. Click on the Team Form button at the bottom of the screen.
3. Fill out the Team Registration Form
• I. GENERAL INFORMATION:
• Fill in RESIDENT ASSOCIATION INFORMATION
• State Association:
• Select Virginia Futsal
• League Association:
• Select No League Association
• Club Association:
• Select No Club Association
• NON-RESIDENT ASSOCIATION INFORMATION
• Leave this section set to the default values.
• TEAM INFORMATION
• You must fill all of the required information in “RESIDENT ASSOCIATION INFORMATION” prior to doing anything in this section. The information in “RESIDENT ASSOCIATION INFORMATION” is used to populate the pull down menus in this section.
• Team Name:
• Type in the Team Name
• Team Type:
• Select the Team Type for your team
• Age/Gender Group:
• Select the Age/Gender for your team
• Category:
• Select the category that accurately describes your team
• You must select at least one season that your team participates in:
• Check Winter Season
• Check Summer Season
• II. PRIMARY TEAM CONTACT INFORMATION:
• If the coach or the assistant coach is the primary contact:
• Enter the coach’s/assistant coach’s number on the line marked Team Contact ID:
• Leave all of the other fields in this section blank
• If someone other than the coach or assistant coach is to be the Primary Team Contact:
• Leave “Team Contact ID:” blank.
• Fill in each of required fields in this section
• You must enter information in each of the required fields or you will not be able to save the form
• The current release of the software will be changed shortly. If you do not see a field under Team Contact ID: field that lets you identify the contact as the Coach/Assistant Coach/Team Manager then only the id of a previously registered Team Manager may be used. In the interim to make the coach the Primary Contact will require re-entering all of the coaches information in the Team Contact fields.
• III. COACH INFORMATION:
• Enter the coach’s number on the line marked Head Coach ID:
• Enter the assistant coach’s number on the line marked Assistant Coach ID: only if you have registered an assistant coach
• Press Submit Team Info Button
4. Print Team Confirmation form
• This form has:
• The Team Name
• Team Id
• Team Password

Requirements for On-Line Registration:

• Make Sure Your Computer & Browser Meet The Following Requirements:
• You will need current browsers
• Internet Explorer 5.01 or newer
• Netscape Communicator 4.08 or newer
• Do not use Apple/Macintosh Computers to register online.
