Coach On-Line USFF Registration
- Click on the preceding link. It will take you to the Welcome to “U.S. Futsal Coach/Instructor Registration” form.
- Fill out the “U.S. Futsal Coach/Instructor Registration” form.
- Select a New/Returning Status:
- Select ’New Coach/Instructor’ if you did not register last year
- Select ‘Returning Coach/Instructor’ if you did register last year
- Press the Register OnLine Button
- If you selected ’New Coach/Instructor’ then proceed to step 5
- If you selected ‘Returning Coach/Instructor’:
- Fill in I. COACH/INSTRUCTOR INFORMATION:
- First 3 Letters of First Name:
- Last Name:
- Birth Date (mm/dd/yyyy):
- Press Submit Coach/Instructor Verification
- If you receive the Verification Process Complete Screen
- You are not in the USFF database.
- Return to Step 2 and select ’New Coach/Instructor’ for New/Returning Status:
- Fill in/Review the Coach/Instructor Registration

- For Present State Association: Select Virginia Futsal
- For League Association: Select No League Association
- For Club Association: Select No Club Association
- III. EXPERIENCE INFORMATION:
- Enter information in all sections of the table. If you do not have experience, enter a ‘0’
- You will not be allowed off of the form until you have entered information in all the sections of the table
- IV. RISK MANAGEMENT INFORMATION:
- Enter information in all sections. If you do not have experience, enter a ‘0’
- Enter None for Position if you previously entered a ‘0’ for that section
- You will not be allowed off of the form until you have entered information in all the sections of the table
- V. IMPORTANT POLICY INFORMATION:
- Read the information in this section and make sure to check each box to indicate your agreement.
- WAFL will perform a Kid Safe process on each coach
- Press Submit Coach/Instructor Application Button
- Review the Coach/Instructor Registration Summary
- Print the Summary, it contains your U.S. Futsal Id
- If you made a registration error, please print the appropriate pages, continue to register, and THEN send an email to registrar@4futsal.com to correct the error. It is important that you conclude the registration process before e-mailing the correction.
- Press Proceed to Payment Solution Button
- Fill out Online Payment Form:
- Options are:
- Online Credit Card
- Mail-in Check
- Press Proceed to Secure Payment Page
- If you selected:
- Online Credit Card then proceed to Step 9
- Mail-in Check then proceed to Step 11
- Fill out Online Payment Form
- Press Proceed to Secure Payment Page
- Print out Confirmation Page
- Congratulations, you are done.
- Print Certificate of Intent
- Registration is not complete until:
- you send a copy of the Certificate of Intent
- a check for $12.00 to:
- U.S. Futsal

- Your registration will be reviewed upon receipt of payment and you will be notified by email once your registration has been approved.
Requirements for On-Line Registration:
- Make Sure Your Computer & Browser Meet The Following Requirements:
- You will need current browsers
- Internet Explorer 5.01 or newer
- Netscape Communicator 4.08 or newer
- Do not use Apple/Macintosh Computers to register online.
